Founded in 1999, Communities and Health Professionals Together (CHPT) began as an informal service-learning partnership between the UC Davis School of Medicine and several community based organizations. CHPT aims to improve child health outcomes throughout the Sacramento region by 1) increasing early career pediatricians' understanding of the social determinants of health and 2) providing tools for engaging in reciprocal partnerships with local organizations via the lens of Asset Based Community Development. In February 2014, CHPT partnered with a local community clinic to create a patient resource desk. The purpose of the service is to help pediatricians identify families impacted by social determinants of health and connect those families with appropriate resources. The desk is staffed by undergraduate volunteers and operates under a case management model. Volunteers make referrals to local organizations, follow-up with families until need is met, and provide regular updates to the physician throughout the process. A VISTA member is needed to further develop the program and implement sustainability strategies.
