This position, working under the supervision of the Director of Managed Care Incentive Program, is responsible for facilitating Dignity Health Medical Foundation's Pay for Performance (P4P) initiative through the development and implementation of processes to meet the P4P best practice standards. These relate to the preventative and chronic disease management clinical practices as captured within the electronic medical record. These activities will include identification and outreach (both verbal and written) to patients requiring preventive and chronic care screenings, patient education, collaboration with providers and staff to identify system barriers to meeting the standards and development of plan of actions and programmatic development. The other key function is to monitor the organization's outcome performance on an ongoing basis and identify issues that need performance improvement. This position works closely with providers and provider staff to support Care Management activities and optimize the quality of patient care. The Program Coordinator will use data from Dignity Health systems including IDX, Managed, AIM, EBIDS, Clinical Warehouse, Allscripts and Care Connect (EMRs) and registries. This position will serve as a liaison to regulatory and certifying bodies (eg NCQA, CMS, etc.) related to supporting communication of related application and data activities.?Requirements: -3 years related experience in the medical field required -High School diploma required. -Detail oriented with strong interpersonal and customer service skills. -Computer experience and comfort is required. -Strong data management skills. -Ability to handle multiple detail oriented and time sensitive assignments, plan and organize work to make effective use of time and ensure mandated deadlines are met, and work effectively with minimal supervision. -Candidate must establish and maintain strong and positive working relationships with internal and external customers while working in a team environment, communicating effectively with technical and non-technical staff and working as an active listener. -Ability to communicate effectively by telephone, in person and/or writing with a wide variety of people.?Preferred Qualification:-Patient and/or clinical experience (MA or LVN) preferred. -Associate's or Bachelor's Degrees and Electronic Medical Record experience are preferred -AllScripts knowledge preferred Dignity Health Medical Foundation, established in 1993, is a California nonprofit public benefit corporation with care centers throughout California.
